That's one of my all-time favourite scare scenes, and from the "Drop of Water" story in the anthology.
It's not a puppet (well, in the story anyway). It's the mummified corpse of an old lady, that returns to haunt a nurse that has stolen from her.
Scary as hell in the context of the film, and still a bit shivery on its own.

Black Sabbath is great and it looks fantastic on Blu-ray. It's an anthology film by the great Maria Bava. If you get to watch it, there are two versions on the disc.
